The Total Vision Service Plan enables you and members of your household the opportunity
to purchase optical goods and services at SUBSTANTIAL SAVINGS through our exclusively
negotiated contracts with reputable optical providers. These contracts establish
a fixed fee for almost all of your optical needs, which eliminates the possibility

How to Use the Plan
Upon receiving your application, Total Vision Services will issue your I.D. cards
which you must present at the time of service to any participating provider.
Should you decide to use your own eye doctor and not take advantage of the reduced
examination fees under the TVS PLAN, take your prescription to any of the provider
locations and they will fill it for you at TVS PLAN rates. Remember to present your
ID card at the time of service.
